Your first visit will consist of an examination and consultation so that we may understand your needs and your wants and discuss with you your treatment options. If you have an emergency we will treat it during your first visit.
You may pre-register with us by using our secure on line Patient Registration Form. This will save you time when you come in to our office for your first visit. Please assist us by providing the following information at the time of your first visit:
- If you have medical or dental insurance, bring with you all the information you have about your coverage. This will help you achieve your maximum benefits from your insurance coverage.
- Let us know if you have a medical condition that may be of concern such as artificial heart valves and joints, mitral valve prolapse, rheumatic fever, diabetes, high blood pressure, or if you are on any medication such as heart medications, aspirin, anticoagulant therapy, etc. Some patients may require preventive antibiotics before dental treatment.
